Welcome! We look forward to seeing you at one of our eight outpatient physical therapy clinics which are in the Denver and Colorado Springs, Colorado area. We offer an extensive Back and Neck Pain Relief Program, Pediatric Physical Therapy, Pool Therapy, Post-Surgical Rehabilitation and the latest technologies like Blood Flow Restriction Training, Myofascial Decompression (Cup Therapy), and Trigger Point Dry Needing. Our focus is to help you reach your goals, return to the activities you need to do and love to do pain-free, and give you the tools to Energize, Empower and Elevate!

Starting Your CACC Physical Therapy Journey

If you are not sure physical therapy is right for you, we offer free 20-minute consultations. Click the button above to fill out a form and a CACC representative will call you to set one up.

Insurance

Colorado is a direct access state. This means you may see a physical therapist without a medical referral. However, for insurance reimbursement, you may need a physician referral.

Reimbursement for our services may be handled on a self-pay basis or through insurance.

New Patient Forms

New Patient forms are available for completion through our Patient Portal or upon arrival to a PT clinic on your initial visit. To gain access to the Patient Portal, please reach out to an office nearest you, schedule your appointment, and ask for the Welcome Letter to be emailed to you. For instructions on how to navigate your Patient Portal, see here.

Filling out New Patient forms prior to your first visit will expedite your program and road to recovery. If you cannot access these forms via the portal, please plan to arrive at your appointment 15 minutes before your scheduled time to complete the necessary information.

Your First Appointment

Arrival

We encourage you to arrive 15 minutes prior to your scheduled appointment time to submit your paperwork and provide your insurance information. You can complete the required paperwork in advance and bring it with you to save time.

What should I bring?

You will need to bring the following items for your first appointment:

  • Photo identification* (valid driver’s license, valid state ID, school ID card, or valid passport)
  • Prescription/doctor referral unless using Direct Access
  • Insurance card

What should I wear?

We recommend you wear comfortable clothing that allows you to move around easily for your appointments. Athletic wear, workout attire and gym shoes will work well.

What can I expect at my first appointment?

You can expect your first visit to last for about 60 minutes. Upon arrival, you will be greeted by our Patient Care Coordinator who will ask to see your photo ID, insurance card and any other paperwork that you brought with you. If you were unable to complete the new patient forms, your PCC will provide those to you at this time to complete prior to meeting with your therapist.

Once you are checked in for your visit, your therapist will take you to a private exam room where they will complete a comprehensive evaluation as well as review your medical history, perform diagnostic testing and speak with you to learn more about your condition.

Attendance Policy

To ensure consistent care, CACC Phoenix Physical upholds a cancellation and no-show policy. We monitor cancellation and no-show rates for all patients and assess fees, when necessary, as described below.

A no-show is when you fail to attend your scheduled visit without notifying us. If you no-show more than once, we will assess a $40 fee each time.

A cancellation is when you cancel your scheduled visit with less than 24 hour’s notice. If you cancel more than once, we will assess a $40 fee each time.

If you have continual no-shows and/or cancellations, we may discharge you from the clinic. If you have questions about this policy, please speak to your patient care coordinator.
